KVKK Data Subject Request Form

To exercise your rights as a data subject under Article 11 of the Turkish Personal Data Protection Law No. 6698 (KVKK), you may complete the form below and submit it through the methods indicated.

Your Request

You must specify which of the following rights under Article 11 of the KVKK you wish to exercise:

  1. To learn whether my personal data are being processed
  2. To request information regarding my personal data if they have been processed
  3. To learn the purpose of the processing of personal data and whether they are used in accordance with that purpose
  4. To know the third parties to whom personal data are transferred, within Turkey or abroad
  5. To request the rectification of personal data if they have been processed incompletely or inaccurately
  6. To request the erasure, destruction or anonymisation of personal data
  7. To request that the operations carried out under items 5 and 6 be notified to the third parties to whom the data have been transferred
  8. To object to a result arising against you through the analysis of the processed data exclusively by automated systems
  9. To claim compensation if you have suffered damage due to unlawful processing of your data

Submission Methods

  • By e-mail: Complete this form and send it to [email protected] with "KVKK application" as the subject line
  • By post: Send the wet-signed petition and a copy of your ID document to Milli Kütüphane Cd. İ.Tepeköylü İş Merkezi No:17/105, Konak/İzmir by courier or by registered mail with return receipt
  • Through a notary public: Have your application formally served to the address above

Response Time

Your request will be concluded within 30 days at the latest, depending on its nature. If your request is rejected or you consider the response insufficient, you retain the right to lodge a complaint with the Turkish Personal Data Protection Board (Kişisel Verileri Koruma Kurulu).
